The form on this page needs to be completed by every ministry. Ideally it will be completed by the Ministry Leader, however s/he can designate another member of the ministry to complete the form. This information is required to be completed online to assure that all ministries include the same information as part of their description. Once the period for completion of this form has passed, the button link at the bottom of the home page will be “hidden” until it is time for the annual update.
All of the fields with asterisks beside them are required fields. You can complete the entire form online if you want. Don’t worry about typing too much. Even though the fields look small, you can type a lot in them. The form will auto-save the information as you type it in. That means you can leave the form and come back to it later as long as you use the same device and browser you initially used to complete the form. This is good if you need to stop for any reason and come back to it later.
If you are uncomfortable completing the form entirely online, you can print the blank form out first and use it as a template to complete a separate Word document. Then all you will need to do is copy and paste your text from the Word document into the online form fields.
Near the bottom of the form is a file upload button. When you click on the button, you will be given an opportunity to find your ministry roster file on your computer and upload it to the form. The roster ideally will be a Word document or Excel file that includes the name, phone number and email addresses of all of your ministry members. This information will not be posted online. It will be given to the church for their records. (NOTE: This is to be used to upload a roster of ministry members only! Anything else uploaded will be ignored! I promise!)
When you have completed the form, you will need to type the “security” word that appears in the “captcha” box. This is a security measure to make sure a person is completing the form and not an Internet “bot.” After you do that, click on the “Click Here to Submit Your Form” button to submit your information. If you have missed any required fields, the form will not be sent. Instead you will get a message that prompts you to complete the missed fields, which will be highlighted in red. Once the form is succesfully submitted, you will receive a confirmation note and a copy of the completed form will be sent to the Ministry Leader, OGOT, and Karen Clay who will transfer the information to this repository. Please review the form carefully before submission! Whatever is received is what will be posted to the repository!
